College is expensive, that’s for sure. Between paying for tuition, housing, purchasing dorm supplies and your child’s credit card bill you might not have realized that you missed a huge step or in other words another bill to pay. Proxy forms. During your child’s college orientation they must’ve thrown the word around a couple times but it just slipped your mind. Now that your child is 18, legally you are not allowed to view their financial information such as housing and tuition, grades and most importantly health. Even though you are probably still paying the bills, you aren’t allowed to view them. Since your child is 18, which means that if your kids have any health issues or in any hospital (God forbid that happens!), doctors will not share the information with you!
Now if you are an out of state parent or live couple of hours away, it is hell of a scary situation.
To solve this issue, parents need to get a health care proxy form.
When you do a quick google search, the best option they’ll give you is to purchase proxy forms from websites such as MamaBear Legal Forms. However, we found cheaper and faster option.
Healthcare proxy form for the University of Texas
This is good for UT Austin, UT Dallas or any other college in Texas.
Is this better alternative to Mama Bear forms
Mama Bear Legal forms is like Turbotax but for legal and proxy forms. Take a look at health care proxy forms. You’ll see that they’re just two pages that you need to print and sign, not at all complex. If you want to spend $78 for a piece of paper, be my guest.
MamaBear does provide a couple of forms like health and financial but in my experience, most people don’t need both. It is a good, easy to find site and it is very user friendly. However, the main part is to get the forms printed out and signed like any form so this is not something that they will do it automatically for you.
Do you you need to get health care proxy form notarized?
No, it is not required to get this form Notarized. You should sign this in presence of 2 adults. If it makes you feel better, then get it Notarized. It is a common misconception that you need to get it notarized in front of an attorney but that can actually be done through any bank or the university can also do for you. UT Austin charges $11 for this.
After you sign it, keep a copy for yourself and give one to your kid.
What about states other than Texas?
You can search for similar forms in other states or leave a comment and I will search and link the forms. Hopefully this helped you save money, time and gave you less stress.
